Social Referent

The Fondation Balcons du Lac brings together the EMS Les Palmiers, in Montreux, the EMS Résidence Les Laurelles, in Territet, the EMS Les Rosiers in Blonay, two temporary reception centers in Montreux and Villeneuve, as well as adapted housing with support in Blonay. It employs approximately 180 employees across all sectors.

In order to complete our team of SAMS (Structures d’accompagnement médico-sociales) in our LADA (Logement adaptés avec accompagnement) structure in Blonay, we wish to hire for an indefinite period

Your mission :

As a social referent, you are a reference person for the tenants and actively contribute to enabling them to live at home in a secure, convivial environment that promotes autonomy.

In this context, you:

• Welcome new tenants, inform them about the functioning of the LADA, and support them in their installation ;
• Develop a relationship of trust based on listening, availability, and respect for each person's autonomy ;
• Identify the needs or difficulties encountered by tenants and direct them, when necessary, towards the appropriate benefits and services ;
• Ensure coordination with the various partners of the social, medico-social, and health networks, notably the CMS and other home service providers ;
• Ensure a regular presence with the tenants and contribute to offering them a secure living environment ;
• Promote social relations, conviviality, and the participation of tenants in the life of the residence ;
• Design and organize socio-cultural activities and animations adapted to the interests and needs of the tenants and contribute to prevention and health promotion actions, notably around fall prevention, physical activity, nutrition, safety, or the maintenance of social ties ;
• Promote self-organization and the involvement of tenants in collective life ;
• Support neighborhood dynamics and intervene as a resource person in case of difficulties or conflicts, prioritizing prevention and mediation ;
• Provide, when necessary, occasional administrative or practical support, respecting the limits of the function and in complementarity with existing services ;
• Ensure the management and use of the community space, notably in connection with the proposed activities ;
• Participate in supervision meetings within the institution and in continuous training related to the exercise of the function within the LADA.

Your profile (F or H) :

• CFC in socio-educational work or equivalent ;
• Driver's license ;
• Experience in the field of the elderly ;
• Marked interest in supporting elderly people and in maintaining their autonomy :
• Interpersonal skills and ability to create a relationship of trust with people of different backgrounds and needs ;
• Ability to work autonomously ;
• Good organizational skills ;
• Dynamic, open, and benevolent personality, with a real sense of responsibility.
• Good computer skills and especially of the usual professional software in the field (DIR, etc…).
