Spitex Coordinator / Registered Nurse HF/FH (60-100%)

On direct behalf of our client, a sympathetic, renowned and rapidly growing Spitex organization (PA model) with a sympathetic corporate culture, we are looking for an experienced Dipl. Registered Nurse HF/FH for coordination and case management (without nursing activities) to expand the team. You will strengthen (selectable) the team in the city of Bern or in the city of Zurich (workplace selectable, both central locations, parking available).

In this versatile function, you are part of a collegial, dynamic team and, as a competent specialist, you take over case management as well as care planning for the care including preventive support of clients at home. You plan the care including needs assessment, target specifications and deployment planning for the FaGes and caring relatives (PA), who care for the patients on-site. You take over case management and coordination with the health insurance companies, stand by the teams in an advisory capacity and support in the consultation of clients (by telephone, occasionally also on-site - e.g., during status assessments).
Together with your team, you ensure the optimal provision of nursing services and client care. As a communicative personality, you also find active cooperation and the exchange of information with various contact persons (management, FaGes, care team, external agencies such as health insurance companies or authorities) easy. To be successful in this exciting job, you have completed the dipl. Registered Nurse HF/FH or an equivalent further education (or are in training for this). Additionally, you have already been able to gain versatile practical experience in case management and coordination in the nursing sector (ideally including Spitex). You enjoy communicating with a wide variety of contact persons, accompanying nursing cases with your teams and building a professional relationship based on trust. You work with a high degree of independence, are well-organized and possess great introductory ability as well as a healthy dose of humor. Do you also have a driver's license (company car available for occasional visits on-site) and describe yourself as an upbeat, flexible and team-oriented personality?
