We are looking for the CMS de Moudon, a:

ASSISTANT·E EN SOINS ET SANTÉ COMMUNAUTAIRE (ASSC) at 50%-80% #

Mission:

• Collaborate on the medico-social care project (support in the form of help and care) based on respect for the principle of the person's autonomy.
• Participate in monitoring the evolution of the client's situation, in collaboration with the client, the entourage, the interdisciplinary team and the attending physicians.
• Provide clients with services such as basic care, nursing care, household help and family help.

Profile:

• Hold a CFC as ASSC.
• Have, if possible, experience in home care.
• Be available to work during the day, in the evening and on weekends.
• Be able to work autonomously and in an interdisciplinary manner.
• Hold a driver's license and possess a vehicle (possibility of using a professional vehicle).
