For you, Palliative Care means accompanying people in their most vulnerable phase of life with expertise and empathy. For a palliative consultation service in the Canton of Aargau, we are looking for a Dipl. Pflegefachfrau HF/FH or a Dipl. Pflegefachmann HF/FH with experience in Palliative Care at a workload of 60 %-100 %, starting immediately or by arrangement. In this specialist function, you will accompany patients with chronic, incurable, and advanced diseases as well as their relatives in challenging life situations and make an important contribution to professional, empathetic, and holistic care. What awaits you in this responsible task • Advising and accompanying patients with chronic, incurable, and advanced diseases • Supporting and accompanying relatives in complex life situations • Collaboration with interprofessional treatment teams on various wards • Support with distressing symptoms as well as evaluation of initiated measures • Accompanying goal clarifications, decision-making, and living wills • Participation in and co-design of round-table discussions and interprofessional meetings • Support in the organization and coordination of follow-up solutions • Documentation of consultation reports, ward entry logs, and measure plans • Structured induction by experienced colleagues from the palliative consultation service What you bring with you • Completed training as Dipl. Pflegefachfrau / Dipl.
