Case-leading certified nursing professional HF/FH 60–100 % – Spitex

For a Spitex organization, we are looking for a committed certified nursing professional HF/FH with enjoyment in working independently and responsibility in case management, starting immediately or by arrangement.

The position offers a lot of personal responsibility, close interprofessional cooperation, and the opportunity to actively help shape nursing processes.

Tasks

• Assumption of case management and coordination of the entire nursing process
• Planning, controlling, and reviewing individual nursing care
• Cooperation with relatives, doctors, and other specialist agencies
• Professional guidance and support of the nursing team

Requirements

• Degree as a certified nursing professional HF/FH
• Independent and responsible way of working
• High professional and social competence as well as good organizational talent
• Experience in Spitex and knowledge of interRAI an advantage
