FOR OUR CMS OF COSSONAY, WE ARE RECRUITING A/AN: RAI Nurse 60% from October 1st RAI Nurse 60% Cossonay YOUR MISSION As a RAI nurse, your role involves advanced mastery of specific skills: • Global assessment: Carry out complete assessments using standardized tools to adapt interventions to the client's needs. • Interdisciplinary coordination: Ensure the link between the client, the multidisciplinary team and the network partners. • Physical and psycho-geriatric care: Provide adapted care taking into account the physical and psychic dimensions related to advanced age. • Community approach: Integrate interventions into a global vision of community health, in connection with the client's environment. YOUR PROFILE • You hold a HES diploma in nursing or an equivalent title, ideally supplemented by at least two years of experience, preferably in home care. You demonstrate a real interest in home care maintenance and the support of elderly people. • Autonomous, responsible and attentive, you demonstrate versatility and possess a great capacity for analysis, negotiation and stress management. Your sense of organization as well as your ability to anticipate needs and adapt to various situations are essential qualities for the position. • You have followed the iHC/ RAI assessment process training or possess knowledge in this method. As a RAI evaluator nurse, you will carry out assessments and provide nursing care to our home care clientele. • You are flexible, mobile and holder of a driver's license, ideally with a personal vehicle.
