The specific field of the liaison nurse covers the imad client base (known or future clients) hospitalized, requiring a return home with imad services. Their field of activity is notably declined in terms of needs assessment, medical validation of required services, predictive coordination of interventions by home maintenance teams (EMD) and/or centralized services. Your missions and responsibilities Inform clients or future clients about the home services offered by imad, the partners of the Geneva socio-sanitary network, the rates and reimbursement terms for home maintenance services. As needed, direct the client to the person or structure that can advise them. Ensure coordination between imad and the private/public hospital sector to organize the return home of clients, with services provided by imad. Facilitate the exchange of useful and necessary information for client care (according to planned documentation). Facilitate hospital discharge and promote a return home in the best conditions. Collaborate with the client, caregiver, health and social professionals. Prevent inappropriate (re)hospitalizations. Work in close collaboration with the imad arcade team located at HUG. Bachelor HES-SO in nursing or equivalent training registered by the Swiss Red Cross Knowledge of the care process and clinical pathways, the Geneva health social network Ability to work alone and in interprofessionalism within a multidisciplinary team Sense of responsibility and solution-oriented, sense of communication, negotiation, and relationships with others Ability to adapt and delegate Stress and conflict management Resistance to physical work.
