Les Peupliers ensures 7j/7j replacements during the day, evening and night at the 7 CMS of the Fondation soins Lausanne in order to guarantee the continuity of care for our clients.

In order to complete our teams, the CMS of the Fondation Soins Lausanne are looking for, immediately or by arrangement, a/an:

Night Community Health and Nursing Assistant

Your mission

• Administer delegated medico-technical care and basic care in close collaboration with the nurse for quality care of clients
• Actively contribute to the clients' maintenance at home through prevention and health promotion measures while taking individual needs into account with personalized care and by promoting client autonomy
• Participate in the development of the clients' therapeutic project within the interdisciplinary team by collecting and transmitting your observations and proposals
• Participate in development projects and working groups around the continuous improvement of services in the field of home help and care
• Actively collaborate in the activities of interdisciplinary teams with a specific role allowing you to value your profession as an ASSC as a full stakeholder in the health system

Your profile and your skills

• CFC as ASSC or a title deemed equivalent (foreign diploma recognized by the CRS)
• Experience in the medico-social field and a strong interest in community health
• Ability to collaborate in a network and multidisciplinary manner
• Sense of responsibility and professional ethics
