Health Specialist / Spitex Coordinator (60-100%)

On behalf of our client, a sympathetic, renowned, and strongly growing Spitex organization (PA model) with a sympathetic company culture (workplace City of Zürich or City of Bern can be chosen), central locations, parking spaces in front of the house), we are looking for an experienced health specialist for the coordination and case management in care for the expansion of the team. Health Specialist / Spitex Coordinator (60-100%)# In this versatile function, you are part of a collegial, dynamic team (Dipl. Nursing Specialist/FaGe/PA in care) and, as a competent specialist, take over the accompaniment/coaching of the nursing relatives (PA), who take over the care/nursing of the clients on site. You work in case management, are on site for needs assessments, and support the PA with regular visits to clients and in their training/instruction. You support the documentation of cases, coordinate billing with health insurance companies, and ensure optimal client care as well as the provision of care services. As a communicative person, you also find it easy to work together actively and exchange information with internal departments (deployment planning, Dipl. nursing colleagues) as well as external contacts (authorities, health insurance companies, caring relatives, and care team). To be successful in this exciting job, you have completed the further training as a dipl. health specialist (FaGe) or equivalent further training (or are in training for it). Additionally, you have already gained versatile practical experience in coordination in the care sector (ideally including Spitex). You enjoy supporting different people in everyday life, instructing and accompanying relatives in care, and building a professional relationship based on trust. You work with a high degree of independence, are well-organized, and have a great ability to adapt, as well as a healthy dose of humor.
