Our social service ensures the interface between patients, their loved ones and the intra or extrahospital therapeutic team Your missions • Participate in the organization of the patient discharge project in a multidisciplinary approach • Evaluate the psycho-social situation of patients and implement the necessary interventions • Promote collaboration with the cantonal and extracantonal socio-sanitary network Your profile • Bachelor of Arts HES-SO in Social Work with a focus on Social Service or equivalent title • Hospital experience and/or knowledge of the Neuchâtel health network, an asset • Good legal knowledge and social insurance • Mastery of usual computer tools (MS office) Your skills • Relational ease and sense of communication • Ability to work autonomously, in a multidisciplinary team and in a network • Flexibility, good adaptability and reactivity • Resistance to stress and pressure • Critical sense, ability to analyze quickly and synthesize Additional information • Place of work: La Chaux-de-Fonds site + mobility on other sites (Pourtalès, Le Locle, and Val-de-Ruz) • Entry into function: immediately or to be agreed upon in an indefinite-term contract • Classified in chain 403, class 7, according to the CCT santé 21 salary scale • Information: Madame Gloria Medina Bernal, Head of Social Service, +41 79 559 50 29
