ASPMAD IS: € 9 CMS in the Nord vaudois € 950 engaged collaborateur-rice-s € More than 6'700 people accompanied each year € A presence in 105 communes FOR OUR CMS OF ORBE, WE ARE RECRUITING A/AN: REFERENCE NURSE AT 80 % EM-Reference Nurse 80% in Orbe YOUR MISSION As a reference nurse, your role involves advanced mastery of specific skills: € Personalized planning: Propose, organize and coordinate intervention plans adapted to clients, ensuring their satisfaction and respect for their needs. € Rigorous follow-up: Supervise and regularly reassess situations, coordinate actions essential to the safety and continuity of care. € Document management: Responsible for the management of OPAS forms and the updating of client files in accordance with directives. € Interdisciplinary collaboration: Work closely with the entire network (CMS team, family caregivers, doctors, partners) to ensure optimal care. YOUR PROFILE You hold a HES nurse diploma or an equivalent title, reinforced by significant experience of 2 years, preferably in home care. You are versatile, attentive and autonomous; possess excellent interpersonal and negotiation skills and show a pronounced interest in maintaining home care. Your sense of organization, your ability to anticipate needs and to adapt to any type of situation and your competence in stress management, are essential qualities.
