Bachelor HES in nursing or a diploma recognized as equivalent by the Croix-Rouge. 3 years of experience in the field of care and physical disability. PURPOSE OF THE FUNCTION. Ensure, in residence, within a multidisciplinary team, personalized care and support services, based on evidence, for adult people with physical disabilities, with associated pathologies and/or cognitive disorders. The objective is to contribute to maintaining their health, quality of life, and self-determination, while ensuring continuity, safety, and quality of support. To this end, the function consists of: Evaluating the health needs of residents, ensuring clinical follow-up and therapeutic surveillance. Accompanying residents in daily life activities, promoting the development, maintenance, or compensation of their abilities, with a view to promoting autonomy. Participating in the development, implementation, and evaluation of personalized projects, ensuring respect for the wishes and objectives defined by each resident. Ensuring a reference role, clinical support, delegation, and advice for ASSC, AS, and ASE. Coordinating the interventions of different professionals and network partners to ensure complementary and efficient support. Identifying, evaluating, and managing complex situations, emergencies, and unforeseen events. Contributing to the daily organization of work and the coordination of the multidisciplinary team. Contributing to the achievement of institutional objectives and the continuous improvement of the quality of support, promoting a multidisciplinary and person-centered approach. YOUR PROFILE.
