To strengthen our team of doctors and Nurse Clinical Practitioners (NCP) in the Department of Internal Medicine, we are looking for a committed person to start immediately or by agreement. In this function, you will play a central role in the care of our patients and work closely with the medical service and other professional groups. You will accompany patients throughout the entire diagnostic and treatment continuum and bring your clinical expertise into innovative care models. #### Advanced Practice Nurse (APN) / Klinische_r Fachspezialist_in 80-100% # Your tasks #### • You will support the medical teams on the ward stations of the Department of Internal Medicine in an interprofessional team. • You will care for patients together with or delegated by senior physicians over the entire diagnostic and treatment continuum. • You will take on clinical, organizational, and administrative tasks in innovation projects such as Hospital at Home and Geriatric Co-Management. • You will participate in tasks related to clinical accompanying research. • You will coordinate care transition in the hospital and the transition from inpatient to outpatient care. • You will conduct pre- and post-stationary clarifications and assess in an interprofessional team whether further measures are required. Your profile #### • You have a degree as MScN, BScN or MAS/DAS Physician Associate. • You ideally already have professional experience as a Clinical Specialist or Advanced Practice Nurse (APN). • You have a category B driver's license.
