Pro Senectute Kanton Zürich is committed as a private, non-profit foundation for over 100 years to the well-being of older people. We are the most important specialist organization for all questions related to age. With a wide range of services and consulting offers, we competently support seniors, their relatives, as well as authorities and institutions throughout the canton. For our service center Limmattal and Knonaueramt, we are looking for a successor due to the retirement of the current job holder as of 1. November 2026 or by agreement a Koordinator/in CareNet+, 60-80 % ## CareNet+ is a coordination offer for complex care situations. The offer is aimed at people from 60 years old in the Affoltern district, their relatives, as well as participating specialist offices and institutions. CareNet+ ensures the coordinated, case-related cooperation of all service providers in the health and social sector and thus contributes to a coordinated and effective support. Your tasks ### • You bear the operational overall responsibility for the CareNet+ offer and promote interdisciplinary cooperation in the district. • You advise and accompany demanding case constellations and ensure a coordinated, needs-based support. Your interventions take place promptly and, if necessary, on site. • You take over the case management and control according to case management principles and are responsible for the documentation in the case management system GERAS. • You identify and develop new cases independently and actively develop the offer further. Your profile ### • You have a completed education in the social sector (FH) or a equivalent tertiary degree. • You have several years of professional experience; experience in case management is desired. • You think in a networked way, work in a structured and independent manner, and keep an overview in complex situations. You are characterized by a high customer orientation. • As a responsible and communicative person, you appreciate the exchange with people and act reflectively and solution-oriented. • With your consulting competence and winning manner, you manage to convince different stakeholder groups and develop viable solutions together.
