The establishment has 240 acute care beds, a maternity ward with around 1,500 births per year, and nearly 30,000 emergency consultations. YOU HAVE • A Bachelor HES in nursing or a foreign nurse diploma recognized by the Swiss Red Cross • Training in community health or a CAS in care coordination and network work, an asset • Very good knowledge of the Vaud health system or a particular curiosity related to this function • A true professional positioning and solid communication, negotiation, and argumentation skills • A strong sense of listening and excellent relational skills • A good overall vision of situations combined with an analytical mind allowing to establish links and consider the entire project orientation issues   YOUR OBJECTIVES • Evaluate clinical situations to build patient orientation • Ensure liaison for the realization of the patient's orientation project, taking into account their wishes and needs, in coordination with the care network partners (relatives, doctors, hospitals, CMS, etc.) • Collaborate with the BRIO coordination nurses for orientation projects in EMS and with the interdisciplinary team to consolidate the orientation project • Organize and lead network interviews • Participate in interdisciplinary colloquia   WE ARE • An institution comprising acute care and rehabilitation establishments, a private clinic, EMS, medical centers, and dispensaries spread across the La Côte region as well as the 1st clinical hypnosis school dedicated to the care environment in French-speaking Switzerland. • A care network allowing many opportunities for internal mobility according to the aspirations and development projects of the employees. • More than 2000 employees taking care of patients and residents in the region, sharing values of Responsibility, Proximity, Humanity, Competence, in an agile environment favoring project implementation. • A training company committed to training the next generation, with around 80 apprentices in 16 different trades, 200 HES-ES interns, and nearly 300 doctors in training.
