Pflegefachfrau/Fachmann HF/FH for Reha- Transition Care and Palliative Care Your mission: You want to accompany people on their individual path - after a hospital stay, a health event, or in a palliative phase. Our Reha- Transition Care (RÜP) and Palliative Care department combines various forms of care that are coordinated with the individual needs of residents and patients. A versatile field of activity with professional requirements ranging from acute medicine to palliative treatment shapes your everyday work. You accompany people in their rehabilitation and support them in regaining their independence. You accompany residents and patients as well as their contact persons with empathy, professional competence, and appreciation. You work closely with medical partners, the physiotherapy team, activation, and case management.
