Welcome Night Nurse in Medicine 3 - Permanent contract at 80% Particularly recognized for its rapid diagnostic approach and the quality of care, the internal medicine service plays a central role within our establishment. Its mission is to receive patients without a diagnosis, often referred by the emergency service. In a context of developing new care pathways, notably in geriatrics and palliative care, our service is called upon to evolve and strengthen its skills in the management of elderly, polymorbid patients presenting complex care situations. In order to complete our service, we are recruiting a Night Nurse in Medicine 3 - Permanent contract at 80% (ref: 1982) Your mission: In a cutting-edge establishment, you will hold a varied position within a multidisciplinary team including nurses, nursing assistants, and doctors. You will be responsible for the reception of patients suffering from diverse pathologies, as part of global management in the acute phase. In this capacity, you are in charge of: • Settling the patient and presenting administrative and care organization information to them • Collecting and transmitting medical and administrative data and completing the computerized care file • Ensuring and organizing care planning according to priorities and the degree of urgency • Performing care according to medical prescription and in compliance with unit and institutional procedures • Ensuring the maintenance of the Electronic Patient Record • Continuously evaluating the clinical state of the patient and their evolution in an acute situation • Identifying situations of fragility, loss of autonomy, and complexity requiring an interdisciplinary approach; • Participating in the management of elderly and polymorbid patients, as well as the support of patients and their relatives faced with palliative situations; • Collaborating with different stakeholders to ensure the continuity and coordination of care; • Ensuring inter-service coordination for the management of patient movements • Coordinating admissions and discharges: internal transfers, external transfers of patients in collaboration with the health manager. We are looking for a profile meeting the following criteria: • Nursing diploma • In possession of Red Cross recognition if foreign diploma • Having experience in internal medicine, geriatrics, palliative care or in a field of complex care, an asset; • Showing a marked interest in the management of elderly, polymorbid and/or palliative patients; • Sensitive to a global and interdisciplinary approach to the patient; • Sense of contact and service within the framework of a private establishment • Autonomy, benevolence, versatility, adaptability, organization, and sense of priorities are essential qualities • Positive attitude within a team and able to generate a climate of trust • Capable of contributing to establishing a climate of trust with patients, their relatives, and various professionals.
