Rigorous monitoring: Supervise and re-evaluate situations regularly, coordinate essential actions for safety and continuity of care. Document management: Responsible for managing OPAS files and updating client records in accordance with directives. Interdisciplinary collaboration: Work closely with the entire network (CMS team, caregivers, doctors, partners) to ensure optimal care. YOUR PROFILE You hold a HES nurse diploma or an equivalent title, strengthened by significant experience of 2 years, preferably in home care. You are versatile, listening and autonomous; possess excellent relational, negotiation skills and show a pronounced interest in home maintenance. Your sense of organization, your ability to anticipate needs and adapt to any situation and your stress management skills are essential qualities.
