2.1 Care and Support • Execution of basic and treatment care according to medical prescriptions and care planning • Professional assessment of care needs and creation of individual care plans • Support of clients in activities of daily living (ATL) • Observation, evaluation and documentation of the health situation • Promotion of independence and resources of the clients • Instruction, advice and support of relatives 2.2 Interprofessional Cooperation • Close cooperation with doctors, therapists and other specialists • Participation in case discussions and internal exchange meetings • Coordination of necessary measures in the care process 2.3 Documentation & Quality Assurance • Electronic care documentation according to internal specifications • Compliance with and further development of quality standards • Participation in quality circles, team development and continuing education 2.4 Organization & Independence • Independent tour planning within the framework of deployment planning • Ensuring efficient and safe work organization • Responsible handling of materials, medications and aids 3. Requirements Professional Requirements • Diploma as Pflegefachperson HF or Bachelor FH • Professional experience in long-term, acute or Spitexpflege is an advantage • Knowledge in somatic care, wound care, medication management • Good observation skills and clinical assessment competence Personal Competencies • High level of independence and sense of responsibility • Reliable, empathetic and resilient personality • Communication and teamwork skills • Flexibility and enjoyment of a varied activity Other Requirements • Driver's license Cat.
