Your area of responsibility You- • accompany and support people with psychiatric illnesses in their home environment • take responsibility for the entire care process, including case management • work closely with internal and external specialists • bring your professional competence and experience into the individual care of our customers • document and plan with interRAI CMH • work independently, responsibly, and flexibly What you bring • Diploma as a nursing specialist HF (preferably with experience in psychiatry) • Enjoyment of outpatient psychiatric care • Empathy, resilience, and a solution-oriented approach • Driver's license category B What you can look forward to • Regular working hours from Monday to Friday • A small, motivated, and humorous team • A lot of personal responsibility and creative freedom • Independent planning of your customer and office appointments • Interprofessional collaboration with our nursing and care teams as well as the departments of dementia, palliative care, and wound management • Regular case discussions, supervisions, and professional exchange Become curious?
