Your main mission will consist of ensuring the continuity of patient follow-up, acting as a reference in the management of their files and providing quality services. By joining the PCLD team, you will be able to: Assume the role of reference in assessing patient needs, in accordance with the ethical principles of the nursing profession, within the home maintenance team model. Provide personalized nursing care, autonomously or by medical delegation, covering a full range of preventive, curative, educational, and palliative services, directly at the homes of patients of all ages. Work in close collaboration with community care and health assistants as well as multidisciplinary aides, delegating care according to established procedures. Organize medico-social steps and ensure coordination with all stakeholders involved in a given patient's situation, by mobilizing the available resources of the primary and secondary networks. A Bachelor HES-SO in nursing or training deemed equivalent, as well as recognition from the Croix-Rouge. An excellent capacity for adaptation and delegation, with demonstrated flexibility and availability. A sense of responsibility and a solution-oriented approach, essential for meeting the challenges encountered. An aptitude for working autonomously and interprofessionally within a multidisciplinary team.
