Your profile

• Holder of a diploma from a recognized social school such as HES (social work track with orientation in social services), EESP-Ecole d'études sociales et pédagogiques Suisse, Bachelor in social work or a title deemed equivalent
• Good knowledge of social insurance and the Vaud medico-social network
• Ability to organize oneself autonomously and ease of adaptation
• Listening and negotiation skills
• Driver's license indispensable

Your little +

• Knowledge of the home care field
• At least 5 years of experience in the medico-social field

Your future missions

• You accompany patients and their relatives to carry out specific and complex procedures for therapeutic purposes by relying on the helping and supporting relationship
• You ensure a liaison and orientation activity with relatives and the medico-social network
• You are the reference partner of the multidisciplinary team for social issues
• You provide a center of reference and expertise to members of the multidisciplinary team for questions relating to legal, insurance, and administrative fields
