Its offering ranges from classic basic and treatment care to specialized psychiatry and wound care, as well as palliative care. Your area of responsibility: • Ensuring resource-oriented, comprehensive care and support in the home environment • Participation in the Palliative Care Team • Deployment in the area of treatment and basic care, as well as needs assessment RAI HC • Case responsibility and ensuring the entire nursing process • Management of the electronic client documentation • Contact with relatives as well as other involved agencies • Interprofessional cooperation • Shifts in both day and evening service Your profile: • Degree as Dipl. Pflegefachperson HF, preferably with additional training in Palliative Care • Professional competence and an independent way of working • High social competence, flexibility, and teamwork skills • Good German skills, oral and written • Driver's license Cat.
