your tasks:
Assessment and counseling of new clients and you accompany them professionally from the beginning.
You regularly check the nursing situation and adapt it individually within the framework of reassessments.
You actively promote the health, independence, and resources of the clients.
You work in a structured manner with the software and ensure seamless documentation.
You coordinate cooperation with general practitioners, insurance companies, and other partners.
You provide advisory support to relatives and assist them in everyday nursing care.
Requirements:
Completed training as Dipl. Pflegefachfrau HF/FH or equivalent recognition (SRK).
2 years of professional experience is an advantage, ideally in Spitex or long-term care.
Independent, responsible, and structured way of working.
High social competence as well as strong communication skills.
Good German language skills in speaking and writing.
Driver's license category B required.
