For efficient coordination, we are looking for a professionally and socially competent person as Social Worker 60-100% - Acute Hospital ## Exciting challenges await you ### • Consulting, caring for, and supporting patients and their relatives in partly difficult life situations • Mediating and organizing doctor-ordered rehabilitation and spa stays • Supporting, consulting, and organizing transitions to old-age and nursing homes • Consulting and supporting in planning ambulatory care at home (e.g., Spitex, meal services, etc.) • Mediating and consulting on institutions and offers in the area of social, addiction, and disability services • Coordinating with relevant external specialist offices, authorities, and municipalities • Participating in interdisciplinary reports, visits, working groups, and projects • Planning and organizing patient transports • Recording patient statistics in the own area of responsibility What distinguishes you for the job ### • Specific training and further education in the field of social work/healthcare; relevant professional experience in a comparable institution of the health or social sector is an advantage • Good knowledge in relevant questions of health, accident, and social insurance law • Empathetic, balanced, and resilient personality with pronounced social skills • Good oral and written communication skills; stylistically secure in German in word and writing • Independent, responsible, and structured way of working • Good administrative and IT skills 6 reasons to work with us ### • Varied workplace and an exciting and challenging activity in a dynamic team • Working hours from Monday to Friday • Possible increase in employment percentage from 2026 • Excellent staff restaurant and numerous discounts • Central location near Uster train station and highway access • ZVV-BonusPass or cheap parking options, max.
