For one of our major clients in the socio-sanitary sector, we are looking for a motivated and autonomous Social Assistant (m/f) at 60-80% to strengthen their home care teams in the Nord Vaudois region. Social Assistant 60-80% (M/F) Job Type: Fixed-term temporary mission of 3 months Activity rate to be agreed between 60% and 80% Mission description: Identify and assess the specific needs of clients in order to carry out adapted social interventions, including personalized follow-up. Provide advice and support to caregivers, while offering strategic guidance to improve client well-being. Collaborate effectively within a multidisciplinary team and with the external medico-social network for a coordinated approach. Actively manage client files by acting as the social reference person. Requirements: Diploma as a Social Assistant or a recognized equivalent qualification. Ideally 2 years of experience in Switzerland. Solid knowledge of social insurance and the socio-sanitary system of the canton of Vaud. Mastery of administrative procedures with ease and confidence. Autonomy, versatility, and great capacity for listening and communication. Driver's license indispensable. Assets: Knowledge of Medlink software. Certifying training in social insurance.
