Introduction

For a modern and growing Spitex organization in the Basel region, we are looking for a dedicated Dipl. Pflegefachperson HF/FH on behalf of our client.

Tasks

• Advising and accompanying persons in need of care as well as their relatives in the home environment
• Carrying out initial assessments for clients on-site, including needs assessment and nursing assessment
• Ensuring professional, empathetic, and trusting initial contact
• Holistic analysis of complex care situations as well as initiation of suitable nursing measures
• Creation, implementation, and review of individual nursing plans
• Preparation and coordination of the required nursing prescriptions
• Ensuring complete and structured nursing documentation
• Coordination and cooperation with doctors, health insurance companies, as well as other internal and external stakeholders
• Shared responsibility for quality assurance and continuous further development of the nursing processes
• Active participation in the optimization of processes and structures
• Promotion of a constructive and appreciative team culture
• Continuous professional and personal further education

Profile

• Completed training as Dipl. Pflegefachperson HF/FH
• Ideally experience in outpatient care (Spitex)
• Strong professional nursing judgment and analytical skills
• High empathy as well as a respectful and professional manner with clients and relatives
• Enjoyment of advisory activities and direct customer contact
• Strong communication skills as well as a confident and trustworthy appearance
• High social competence and the ability to act in a solution-oriented manner even in demanding situations
• Interest in helping to shape processes and actively helping to build new structures
• Openness to change and enjoyment of a dynamic working environment
• Independent, structured, and responsible way of working
• Teamwork skills, flexibility, and commitment
• Good digital knowledge, ideally experience with electronic nursing documentation systems (advantageous: Perigon)
• Very good German skills in speaking and writing; further language skills (French, Italian, or Portuguese) are an advantage
• Driver's license category B
