The goal is to organize a suitable, timely inpatient follow-up solution for our patients, within the framework of rehabilitation, spa stays or nursing home admissions. In doing so, we rely on close exchange between doctors, nursing staff and relatives. For us, the optimal care of the patient with simultaneous cost control of hospital stays is the priority. Then come on board!   YOUR ADVANTAGES WITH US • A workplace in the heart of Bern • Paid changing time • Contribution to mobility costs • Discount at the Salem-Apotheke • Contribution to fitness subscription   Here you can take a closer look at which benefits you can benefit from: Teamdiagnose.ch YOUR TASKS ## • You organize timely, suitable inpatient follow-up or transition solutions for our patients • You advise and accompany patients and their relatives during the entire discharge management process • You coordinate the transfer of patients to other institutions together with internal and external bodies • You work constructively with the interdisciplinary teams • You actively participate in continuously optimizing the cooperation with our external bodies (social insurance, various clinics, homes, KESB, etc.) • Through the support of the DRG team, you have a data basis to make our discharge management as efficient as possible YOUR PROFILE ## • You have completed basic medical or nursing training as well as professional experience in Case Management - ideally supplemented with further training in Case Management • You are characterized by a sense of responsibility and an independent, flexible and loyal personality with strong organizational talent • Through your solution-oriented as well as networked way of thinking and acting, you understand connections and can act accordingly • Thanks to your pronounced social competence and your communication skills, dealing with various stakeholders is easy for you • We require proficient German language skills and every further language is an advantage • You are experienced in using Microsoft Office   For further information, please feel free to contact Salome Würgler, Deputy Head of Case Management directly at T +41 31 337 61 00 / salomependa.wuergler@hirslanden.ch.
