Specialized Nurse in Patient Case Management (a) 80-100% # Specialized Nurse in Patient Case Management (a) 80-100% YOUR TASKS • You advise and inform on social, personal, financial and legal issues related to the hospital stay and discharge planning. Additionally, you organize tailor-made follow-up solutions, provide material assistance if necessary and direct towards the appropriate specialized services • You maintain contacts and cooperation with external institutions and providers • Close interdisciplinary collaboration with the entire medical, nursing and therapeutic team awaits you • You play an active role in the design and continuous development of the case management service for our patients YOUR PROFILE • Completed training as a registered nurse with experience • Ideally, you have experience in case management in an acute hospital or additional training in case management • Good knowledge of the healthcare system and social insurance • Empathic and integrative personality with high social competence • You like working independently but also in a team.
