Your tasks ###

• Advice and professional support for family caregivers - by telephone, digitally, as well as during home visits
• Holistic assessment of individual nursing and support needs
• Development of suitable solutions together with clients and their relatives
• Long-term support and assistance with the goal of promoting independence and quality of life
• Close cooperation and coordination with family doctors, therapists, insurance companies, social services, and other specialist agencies
• Participation in case discussions and internal team meetings
• Careful documentation of nursing processes, services, and observations in the electronic system
• Ensuring correct service recording
• Participation in the further development and optimization of nursing and care processes
• Involvement in internal projects, quality developments, and training

Your profile ###

• Completed training as a Dipl. Pflegefachperson HF/FH or an equivalently recognized qualification
• Experience in outpatient care, nursing advice, or family caregiver care is an advantage
• Enjoyment of counseling and personal contact with clients and relatives
• Independent, structured, and responsible way of working
• Strong communication skills and empathy
• Confident handling of digital working tools and electronic nursing documentation
• Good German language skills in speaking and writing
• Driver's license category B is an advantage
