Occupancy rate: 80% at our La Chaux-de-Fonds site Your Missions: • Collaborate with the entire multidisciplinary team to achieve the resident's life project; • Engage in a personalized curative, educational or palliative care process that fulfills the resident's life project; • Be the reference person for residents, make the link with the family and partners and participate in their annual assessment; • Analyze situations, identify resources and set goals for resident care in an interdisciplinary manner; • Be the guarantor of the quality of care and participate in the ongoing training of teams, interns and students; • Be an actor in the evolution and updating of the theoretical and technical knowledge of the care team in collaboration with the head nurse • Participate in care situation evaluations and contribute to the use of the PLAISIR tool; • Ensure professional communication with all stakeholders; • Contribute to quality initiatives by applying internal processes. Your Profile: • Nurse diploma, Bachelor of Science HES-SO in nursing or equivalent title recognized by the Swiss Red Cross; • Experience in a similar position; • You are motivated to work in the field of physical, mental and other associated disabilities with a marked interest in maintaining and developing autonomy and rehabilitation; • Dynamic and solution-oriented, you are looking for an environment where teamwork is at the center: • Flexibility: irregular schedules including weekends and on-call service. An excerpt from the judicial record may be requested. This position involves contact with vulnerable people. As such, proof of immunity or vaccination against measles and chickenpox, as well as vaccination against hepatitis B, is required. If necessary, these vaccinations can be carried out by our services and taken care of by our institution.
