La Fondation Saphir, a multi-mission and multi-site foundation (psychogériatrie, gériatrie, psychiatrie adulte), is active in the medico-social field in the canton of Vaud and in the reception and support of persons whose physical and mental integrity is fragile during their life process.
We are looking for our establishment EMS Mont-Riant in Yverdon-les-Bains, which welcomes 56 Persons and is specialized in the field of geriatric psychiatry:

Regardless of their function, the professionals of Fondation Saphir have a common will, that of accompanying Persons on a daily basis to allow them to concretely experience the institutional service strategy "like at home". That is to say, to maintain their habits as much as possible, to offer a response adapted to their wishes and, through real interprofessional collaboration, to allow the well-being and happiness of the Persons.
As an Assistant in community nursing and health, you accompany Persons in the acts of daily life and contribute to their global support, taking into account their physical, psychic, social and spiritual needs. You promote their autonomy and their quality of life, while ensuring to offer them a secure, benevolent environment adapted to their needs.
You provide the care and medico-delegated acts pertaining to your function, under the responsibility of the registered nurse. You observe the health status and the needs of the Persons, transmit relevant information and contribute to the evaluation and adaptation of their support.
You participate in the development, implementation and monitoring of support projects in collaboration with the interprofessional team. You also contribute to the quality of transmissions and the continuity of care. Depending on needs, you participate in the reception and integration of new professionals, apprentices and interns of the support service.
YOUR BACKGROUND - YOUR QUALITIES AND SKILLS
You hold a CFC as Assistant in community nursing and health (ASSC).
You ideally possess experience in the support of elderly persons, particularly in the field of psychogeriatrics.
You are comfortable in the performance of care and medico-delegated acts pertaining to your function.
You adopt an attitude centered on the well-being, needs and resources of the Person.
You demonstrate dynamism, creativity, spirit of initiative and appreciate teamwork.
You are flexible, adaptable, organized and capable of managing priorities.
You have good relational skills and know how to communicate in an adapted manner with Persons, their relatives and professionals.
You appreciate interprofessional collaboration and know how to question yourself in a continuous improvement process.
You possess an excellent command of French (oral and written) and are comfortable with common IT tools, notably Carefolio.
