In close collaboration with the multidisciplinary team and the situation referents, you will play a central role in the implementation and evaluation of care objectives. Your main missions: Perform medico-delegated care, basic care, and household help in compliance with prescriptions and individualized needs. Observe, evaluate, and transmit changes in the health status of beneficiaries in order to adapt the care. Support relatives and families, and contribute to interdisciplinary coordination. Actively participate in the continuous improvement of the quality of care and the sharing of practices within the team.
