FOR OUR CMS OF COSSONAY, WE ARE RECRUITING A/AN: RAI Nurse 60% from October 1st RAI Nurse 60% Cossonay YOUR MISSION As an RAI nurse, your role involves advanced mastery of specific skills: · Global assessment: Perform complete assessments using standardized tools to adapt interventions to the client's needs. · Interdisciplinary coordination: Ensure the link between the client, the multidisciplinary team, and network partners. · Physical and psycho-geriatric care: Provide adapted care taking into account the physical and psychic dimensions related to advanced age. · Community approach: Integrate interventions into a global vision of community health, in connection with the client's environment. YOUR PROFILE · You hold an HES nursing diploma or a title deemed equivalent, ideally supplemented by at least two years of experience, preferably in home care. You demonstrate a real interest in home maintenance and the support of elderly people. · Autonomous, responsible and listening, you demonstrate versatility and possess a great capacity for analysis, negotiation and stress management. Your sense of organization as well as your ability to anticipate needs and adapt to various situations are essential qualities for the function. · You have followed the iHC/ RAI assessment process training or possess knowledge in this method. As an RAI evaluator nurse, you will perform assessments and provide nursing care to our home-based clientele. · You are flexible, mobile and holder of a driver's license, ideally with a personal vehicle.
