The Direction des soins embodies authority in matters of care. Its mission is to guarantee the quality and safety of care within the establishment. In this capacity, the Direction des soins has oversight of all care activities (nurses and various healthcare professional bodies). To fulfill its mission, it relies on three areas of expertise: quality, sustainability and digital innovation; clinical expertise and professional practices; management and transformation. The social service is composed of several committed teams, evolving in an environment rich in interprofessional collaborations. You intervene with hospitalized or outpatient patients facing social, administrative, psychological or family difficulties, requiring a social assessment and close support regarding their care-related situations. Social Worker – 50% As a social worker within the HUG, you intervene with the patients of the geriatrics and rehabilitation department in a global and multidisciplinary approach. You ensure the social follow-up of hospitalized male and female patients, particularly in rehabilitation units and placement waiting units. Your role consists of assessing the social needs of patients and their relatives, identifying social difficulties and proposing adapted solutions. You intervene in the management of administrative and financial requests, and support patients in their procedures for maintaining residence at home, rehabilitation or transition to another establishment (EMS, care structures). You work in close collaboration with the internal multidisciplinary team as well as with external partners. Your work is based on individual interviews, family interviews and multidisciplinary meetings to establish a relationship of trust with female and male patients, understand their situation and develop concrete and personalized solutions. You are also responsible for the rigorous transmission of the social needs and projects of patients to the nursing team and external partners. Finally, you actively participate in professional monitoring of social and legislative themes, particularly social insurance, and in the prevention of social problems. You are involved in the continuous improvement of practices within the service. Qualifications You hold a Bachelor HES in social work or a title deemed equivalent. Holding a CAS as a training practitioner and/or a Federal Diploma in social insurance constitutes an asset. You possess solid knowledge of the social network of the Geneva basin as well as social insurance. Experience in social work in a hospital setting and/or in geriatrics is also an advantage. Equipped with excellent interpersonal skills, you appreciate interdisciplinary work and are comfortable managing complex and emotionally demanding situations. Organized, you know how to manage priorities, demonstrate autonomy in your activity and possess excellent writing skills. Furthermore, you master common IT tools.
