Social Worker for the Social Service in Acute Care (m/f/d)

The Spitalverbund Appenzell Ausserrhoden with the Akutspital Herisau and the Psychiatrischen Zentrum AR in Herisau ensures the expanded basic medical care for the population in the Canton of Appenzell Ausserrhoden and is also the first point of contact for a large part of the population in the surrounding region. Both locations are relevant for care for the Canton and beyond.

In the acute care nursing area under the nursing management of Frau Dragica Novakovic, we are looking for a social worker with a workload of 50% for our in-house social service starting from 01. November 2026 or by arrangement.

Your tasks

• Differentiated counseling and personal support for patients as well as their relatives
• Support in complex situations regarding social and professional integration
• Counseling for and organization of inpatient follow-up solutions
• Electronic data processing in the area of responsibility
• Good networking with other institutions

Your profile

• You have a completed degree (BSc/MSc) in Social Work or are a dipl. Pflegefachfrau HF with a CAS Casemanagement and bring experience in case or discharge management as well as in the hospital and insurance sectors of Switzerland
• You want to work 50% and are ready for holiday substitutions of up to 80 - 100%
• You have very good German skills in speaking and writing
• In addition, you are a committed personality with pronounced social and communicative competence, possess initiative and organizational skills as well as personal stability and resilience
• Independent working methods, assertiveness and enjoyment of interprofessional activity are a matter of course for you
