• You advise patients with heart failure within the heart failure counseling team as part of your advanced nursing competencies and actively participate in shaping the further development of this offering within the LUKS Gruppe
• You care for patients with telemedicine components together in the interprofessional team of integrated care with cardiologists, general practitioners, and Spitex specialists
• You assume a central coordination role in this new care system with telemedicine components and actively participate in its development and implementation as part of a study
• You are actively engaged in the further development of nursing care for patients with heart failure within the LUKS Gruppe and beyond

Your experience ###

• You have a completed Master's degree in nursing as an APN, preferably towards NP, and are interested in heart failure
• You assess complex issues in a structured manner, set priorities according to the situation, and independently derive appropriate measures from them
• You advise employees in a target-group-oriented manner and act professionally and reliably even with medical or organizational questions
• In interprofessional collaboration, you actively contribute your professional perspective and help shape new structures and processes in a solution-oriented manner
