ASPMAD IS: • 9 CMS in Nord vaudois • 950 committed employees • More than 6'700 people accompanied each year • A presence in 105 communes FOR OUR CMS DE SAINTE CROIX, WE ARE RECRUITING A: SOCIAL WORKER 60-90% Social worker 60-90% Ste-Croix YOUR MISSION As a social worker, your role involves advanced mastery of specific skills: · Needs analysis and social intervention: You identify and evaluate the specific needs of clients in order to direct interventions within an adapted social health framework, including personalized follow-up based on the needs of each client. · Support for family caregivers and orientation: You provide advice and support to relatives, while offering strategic orientations to improve the well-being of clients. · Interdisciplinary collaboration: You collaborate effectively within a multidisciplinary team and with an external network for a coordinated approach. · Case management: You actively manage client files by acting as a referent YOUR PROFILE · You hold a diploma as a social worker or a title recognized as equivalent, ideally supplemented by two years of experience in Switzerland. · You have a solid knowledge of the social insurances of the socio-sanitary system of the canton of Vaud. · A certifying training in social insurances would be a plus. · Equipped with autonomy, versatility, and ease of adaptation, you have a marked interest in home care as well as social insurances and the helping relationship. · You demonstrate a great capacity for listening and communication within a group and with clients. · You master administrative procedures with ease and confidence! · You possess a driver's license and, ideally, a personal vehicle.
