Main missions Guarantee the monitoring of the evolution of the clients' situation at home and ensure the coordination of their care project by organizing interdisciplinary interventions, in close collaboration with the client, their entourage, the treating physician, and network partners, in order to ensure personalized, secure, and quality care promoting remaining at home.
