Pflegefachperson HF as Case Coordinator (70-100%) Your tasks In this versatile role, you combine professional nursing with coordination and case management: • You care for and accompany clients directly on-site within the framework of nursing interventions. • You coordinate and organize the provision of services from admission to discharge - in close cooperation with clients, relatives, doctors, hospitals, and other specialist centers. • You take responsibility for the entire nursing process, including needs assessment, nursing diagnostics according to NANDA, and electronic nursing documentation. • You delegate tasks in an appropriate manner regarding the situation and competence and support employees with professional questions. • You take on daily responsibility and contribute to a high quality of nursing care. • You actively participate in the training of apprentices.
