Specialist Residential Support / Needs Assessment 40-60%

What awaits you:

• You independently plan, organize, and create medical prescriptions, needs assessments, and care plans for the clients assigned to you.
• You plan, organize, and accompany the assigned clients according to the mandate and ensure the associated administrative tasks - from assessment through performance recording and evaluation to progress documentation.
• You work throughout the entire Canton Schwyz and accompany our clients in their respective living environments.
• You support and instruct the residential support staff in the implementation of care planning as well as in professional progress documentation.
• You maintain a goal-oriented and appreciative exchange with important internal and external partners, especially with employers, relatives, legal representatives, doctors, and other professional support services.
• You ensure the up-to-dateness of the administrative master data and release declarations of the clients assigned to you.
• You reliably report administrative changes to the Central Services.
• You actively participate in team meetings as well as team and case supervisions and contribute your professional perspective.

What you bring with you:

• Completed training as a dipl. Pflegefachperson HF/FH with the qualification to take over case management including needs assessments or the willingness to obtain these
• Several years of professional experience in psychiatric nursing and needs assessment
• Several years of experience in accompanying adults with mental impairments
• High social competence and strong communication skills
• Independent, structured, and reliable way of working
• Strong planning and organizational skills
• Flexibility and willingness to organize the work week independently and according to needs
• Valid driving license category B and own vehicle mandatory
• Proficient German in speaking and writing
• Confident use of MS Office, smartphone, and digital working tools
• Willingness to work throughout the entire Canton Schwyz
