In order to complete our teams, La Fondation Soins Lausanne is looking for immediately or by agreement several:

Referent Nurse (M/F)

Activity rate between 80% and 100%

Permanent indefinite contract - daytime schedule and some weekends

Your mission

• Accompany clients in their life project at home from a partnership perspective by giving meaning and coherence in close collaboration with the surroundings and loved ones while developing bonds of trust
• Pilot the global and integrative care of clients while relying on internal staff and interdisciplinary teams and in good coordination with the institutions involved in the care network
• Elaborate the therapeutic project taking into account the complexity of care and develop it according to evolution
• Provide assessment, advice, examination and treatment services, as well as nursing care and basic care for quality management of clients
• Contribute actively to the sustainable home maintenance of clients through prevention and health promotion measures while taking individual needs into account with personalized care and by favoring client autonomy
• Participate in development projects and working groups around the continuous improvement of services in the field of home help and care

Your profile and your skills

• Bachelor in nursing or title deemed equivalent (foreign diploma recognized by the CRS)
• Experience in the medico-social field and a strong interest in community health
• Autonomy and decision-making skills
• Ability to collaborate in a network and multidisciplinary
• Sense of responsibility and professional ethics
