Responsibilities Ensure basic care and delegated medico-technical care at the patients' homes. Accompany beneficiaries in daily life activities while promoting their autonomy. Observe the patients' health status and transmit relevant information to the multidisciplinary team. Participate in the development, monitoring, and evaluation of care plans. Guarantee individualized, quality care that respects the patients' needs. Ensure the traceability of care and respect current protocols. Collaborate with the various health professionals intervening at the home. Profile Holder of a CFC as an Assistant in Nursing and Community Health (ASSC) or a recognized foreign diploma (or in the process of recognition) allowing the practice of the profession in Switzerland. Experience in home care is an asset. Autonomous, organized, flexible and endowed with excellent interpersonal skills. Ability to work alone while collaborating effectively with a multidisciplinary team. Good command of French, both spoken and written. Category B driving license mandatory.
