The position is attached to the Direction des opérations, whose missions are to offer optimal conditions for the provision of medico-nursing services, to ensure the integration of the patient journey in collaboration with medico-nursing and administrative partners, and to connect the patient journey and the clinical itinerary in partnership with the Direction médicale and the Direction des organisation, which is divided into 4 entities: the service de Gestion des flux patients, the accueils et la centrale téléphonique, the service de Transport intrasite de la patientèle, and the Service social. Social Worker – 80% As a social worker within the HUG, you intervene with the clientele of the department of geriatrics and rehabilitation in a global and multidisciplinary approach. You ensure the social follow-up of hospitalized patients, particularly in an acute care unit. Your role consists of assessing the social needs of patients and their relatives, identifying social difficulties, and proposing adapted solutions. You intervene in the management of administrative and financial requests, and accompany patients in their steps for maintaining residence at home, rehabilitation, or transition to another establishment (EMS, care structures). You work in close collaboration with the internal multidisciplinary team as well as with external partners. Your work is based on individual interviews, family interviews, and multidisciplinary meetings to establish a relationship of trust with patients, understand their situation, and develop concrete and personalized solutions. You are also responsible for the rigorous transmission of the social needs and projects of patients to the nursing team and external partners. Finally, you actively participate in professional monitoring of social and legislative themes, particularly social insurance, and in the prevention of social problems. You are involved in the continuous improvement of practices within the service. Qualifications You hold a Bachelor HES in social work or a title deemed equivalent. Holding a CAS de praticien formatrice and/or a Brevet fédéral in social insurance is an asset. You possess solid knowledge of the social network of the Geneva basin as well as social insurance. Experience in social work in a hospital environment and/or in geriatrics is also an advantage. Equipped with excellent interpersonal skills, you appreciate interdisciplinary work and are at ease in managing complex and emotionally demanding situations. Organized, you know how to manage priorities, demonstrate autonomy in your activity, and possess excellent writing skills. Furthermore, you master common IT tools.
