Spitex Emmen with its around 60 employees is an innovative, versatile and competent employer in the region. We are looking for a committed and team-capable personality as a dipl. nursing specialist HF / FH 60 - 100% for case management in an interdisciplinary nursing team. Your tasks with us include case management responsibility for the entire nursing process control of a client group with the interRAI HC instrument (including specialist discussions, case discussions, etc.), ensuring holistic support, care and nursing of our clients at home, documentation of nursing-relevant information, cooperation and exchange of information with external institutions such as hospitals, general practitioners, pharmacies, etc., and daily responsibility. What do you bring with you? Completed training as a dipl. nursing specialist HF / FH, further training and experience in handling the interRAI HC instrument are an advantage, empathetic, communicative personality with independent working style, driver's license or willingness to complete missions with the e-bike, IT skills in MS 365 are an advantage.
