Pflegefachperson HF as Case Coordinator (70-100%) Your tasks In this versatile function, you combine professional nursing with coordination and case management: You care for and accompany clients as part of nursing assignments directly on site. You coordinate and organize the service delivery from admission to discharge - in close cooperation with clients, relatives, doctors, hospitals and other specialist agencies. You take responsibility for the entire nursing process including needs assessment, nursing diagnosis according to NANDA and electronic nursing documentation. You delegate tasks in a manner appropriate to the situation and competence and support employees with professional questions. You take on daily responsibility and contribute to a high quality of care. You actively participate in the training of apprentices.
