Certified Nursing Specialist with additional function RAI HC Responsible

Your tasks

• You conduct needs assessments with RAI-HC at the customer's location
• You take over assignments in basic and treatment care for our customers
• You advise and accompany our customers and their relatives
• You carry out RAI assessments and BMF on schedule
• You create nursing documentation and the individual care planning
• Professional support and accompaniment of the Spitex team
• Planning, coordination, and evaluation of nursing assignments

What you bring with you

• Certified Nursing Specialist HF or an equivalent training
• Experience in outpatient care and good knowledge in the area of RAI HC
• Several years of professional experience in long-term care or the Spitex sector is an advantage
• You are resilient, flexible, and accustomed to working independently
• You are a strong communicator and a responsible personality with team spirit
