Nursing Specialist HF Clarification & Consulting 60% or up to 90% in combination with Wound Expert role from October 1st or by arrangement. Your job with us: You professionally clarify the care needs of our clients, control the care process, and ensure that they receive the right support. You are not just part of the care process, you shape it. You are responsible for: Initial assessments and reassessments with RAI HC, assessing complex care situations, creating and further developing care plans, first deployments and handovers in the team, collaborating with doctors, insurers, and internal specialists, and advising clients, relatives, and specialists. Do you want more? Are you interested in wound management? Then you can increase your workload to up to 90%. We will gladly support your further training as a Wound Expert. You bring: Diploma as a Nursing Specialist HF, experience in the care process and with RAI HC, ideally knowledge of root Perigon, analytical thinking and independent working methods, enjoyment of collaboration and communication, good PC skills, driver's license category B.
