Your main tasks ### • You advise and support caregivers in complex and challenging care situations • You conduct clarifications of complex care situations for referrals before hospital admission • You promote the theory-practice transfer and strengthen evidence-based care practice • You initiate and accompany the implementation of findings from care research • You develop care guidelines, concepts, and specialist topics further and implement them together with the teams • You bring your extended clinical competencies into close collaboration with the care teams • You moderate case discussions, work in commissioned groups, and design further training courses • You accompany caregivers in their daily practice and support them in their professional development • You work on care-related and interprofessional projects (e.g., introduction of KIS) and contribute to quality management • You work closely with your PEX colleague, your PEX colleague in Basel • You work closely with the PEX committee and actively bring your professional perspective into it Your profile ### • Completed training as a dipl. care specialist HF or FH • MAS/CAS Gerontology or CAS Intercare • Several years of professional experience in care practice, ideally in a setting with multiple locations • High professional and clinical competence • Enjoyment in initiating development processes and sustainably strengthening care quality • Pronounced communication and consulting competence • Ability to accompany people professionally, motivate them, and convey knowledge in a practical way • Committed, reflective personality with a sense of responsibility and an appreciative attitude What to expect from us Independent work with a lot of creative leeway A professionally challenging role with development potential The opportunity to actively shape the profession of care Collaboration in a motivated and committed team Modern care and support concepts Very good internal and external further training offers Closely networked interprofessional collaboration Do you want to think care further and promote and strengthen people?
