Ensure monitoring of the patients' health status and transmit information to the reference nurses. Participate in the development and implementation of care plans. Accompany beneficiaries in daily life activities. Collaborate with the multidisciplinary team to guarantee quality care and maintenance at home. Your profile CFC as Assistant in Community Health Care (ASSC) or a title deemed equivalent. For foreign diplomas: CRS recognition mandatory. Confirmed experience of a minimum of 2 years in home care. Ability to work independently and manage priorities. Sense of organization, listening, and teamwork. Excellent interpersonal and professional qualities.
